Comprehensive Heart Health Program Planning Workbook, 1998

This workbook, developed at the beginning of the Ontario Heart Health Program Phase I, serves as an excellent foundation document for heart health promotion. It presents steps for a community undertaking long-range heart health planning as well as practical examples from the Heart Health demonstration sites. It links the intervention ‘cube’ and logic model concepts with comprehensive planning.

Comprehensive Program Planning Facilitator's Guide, 1998

This guide highlights a process that could be used by a facilitator to assist a Community Partnership or sub-committee in completing the five program planning steps outlined in the workbook (above). Many tools have been included to assist facilitators in leading groups through these steps. Additional resources have also been identified for facilitators who feel they would benefit from this.

Feature Newsletter on Planning, Vol. 1, No. 4 - Fall 1998 (HTML)

The focus of this issue of @heart is on program planning. It is meant to assist specifically with the creation of the four year plans required of all of Ontario's Heart Health coalitions and, over the next several years, for the annual plans and those related to planning interventions. The stories, tips and resources that are featured here should be of use to many partners within each coalition so sharing the newsletter with others in the community is encouraged.

" Our Stories" Heart Health Action Program: The Experiences and Learnings of the Heart Health Action Program Demonstration Sites, November 1996

An intended companion to the Final Evaluation Report, this 79 page report is a collaborative effort of the volunteers and staff who initiated, developed and maintained the interventions at the five heart health demonstration sites.
